Medical Emergency Information Form

The Medical Emergency Information form is a voluntary program to ensure, in the case of emergency, the EMTs have rapid access to your next of kin (ICE) contact information and your list of current medications.  EMTs need this information in order to be sure any medications they may be required to use for your benefit in an emergency, do not conflict with any other medications you may be currently taking.
